N-648 Medical Disability Evaluation

What is Form N-648?

Form N-648 is a Medical Certification for Disability Exceptions, completed by a licensed medical provider and submitted with a patient’s naturalization application (Form N-400). This form is used when a physical or mental impairment prevents an applicant from meeting the standard English and civics requirements.

Our N-648 Evaluation Process
